The name change procedure in Madhya Pradesh is straightforward. The search results are not. One well-ranked guide cites Articles 19(1)(a) and 21 of the Constitution of Madhya Pradesh, a document that does not exist, and its sample affidavit has you declare that you are a citizen of Madhya Pradesh, which is not a thing either. The same site tells you to advertise in the Times of Madhya Pradesh, a newspaper it appears to have invented. Another writes that the Department of Publication is located only in Madhya Pradesh, so residents must travel there. It is in Civil Lines, Delhi, and that sentence is a find-and-replace over a Delhi guide.

A Gazette notification is a formal announcement of your name change printed in the official journal of a Government. Once it carries a date and a number, passport offices, banks, boards, employers and foreign missions accept it as the legal record of your new name. An affidavit on its own is the beginning of the change, not the end.

Has any state changed its own name and shape more than this one?

Renamed once, capital moved once, a third of the territory gone. Every one of those changes happened the same way yours will.

The Central Provinces were formed in 1861, became the Central Provinces and Berar in 1903, and were run from Nagpur. In 1950 the whole province was renamed Madhya Pradesh, the central province, still with Nagpur as its capital. Next door, the princely states were being stitched into units of their own: Madhya Bharat, created on 28 May 1948 with Gwalior as capital, Vindhya Pradesh with Rewa as capital, and Bhopal State, taken over by the Government of India in 1949.

Then came 1 November 1956. The States Reorganisation Act merged Madhya Bharat, Vindhya Pradesh and Bhopal into Madhya Pradesh, and in the same stroke handed the Marathi speaking Vidarbha and Berar districts, Nagpur included, to Bombay State. The state gained three units and lost its capital on the same day, and Bhopal became the new capital. Forty four years later the Madhya Pradesh Reorganisation Act, 2000 (Act 28 of 2000), which received assent on 25 August 2000, carved sixteen districts out to form Chhattisgarh on 1 November 2000, and Madhya Pradesh stopped being India's largest state by area.

Notice what did the work each time. Not a form, not a fee, not a queue: an official notification, published, dated and open for anyone to check. A province, a capital city and a person all change names through the same instrument. Yours takes weeks rather than a century of redrawing, and it costs a few thousand rupees.

Is there a gazette office in Madhya Pradesh for a name change?

No, and the number of pages telling you otherwise is the reason this section exists.

Madhya Pradesh publishes a state gazette for Government business, notifications and orders. What it does not have is a public window where an individual files a personal name change, and it does not have a citizen portal for one either. Pages telling MP readers to walk their file into a gazette office in the state, or to search Google for the official Madhya Pradesh gazette portal and use whichever site appears first, are describing something that is not there. That advice is how people end up paying a private website that looks official.

For a name change that a passport office, UIDAI, a bank, the MP Board or an employer will accept, you apply to the Central Gazette of India. The file goes by speed post from Bhopal, Indore, Gwalior or anywhere else, and geography costs you a few postal days and nothing more.

Four claims about Madhya Pradesh that are simply false. First, that the Department of Publication is located in Madhya Pradesh and residents must travel there. It is in Civil Lines, Delhi, and that page has clearly run a find-and-replace over a Delhi guide. Second, that you must advertise in a paper called the Times of Madhya Pradesh. No such newspaper exists, and the same site invents equivalent titles for other states. Third, that your right to change your name comes from Articles 19(1)(a) and 21 of the Constitution of Madhya Pradesh. India has a single Constitution and no state has one of its own. Fourth, that an affidavit should declare you are a citizen of Madhya Pradesh. There is no such citizenship. You are an Indian citizen who is a resident of Madhya Pradesh, and an affidavit saying otherwise is a badly drafted affidavit.

Which language should the newspaper notice be in?

One Hindi daily and one English daily. In Madhya Pradesh that copied instruction happens to be correct, and here is the statute behind it.

Most state name change pages are the same template with a place name pasted in, and the line telling you to advertise in the regional language, Hindi is usually wrong: in Punjab the language is Punjabi, in Odisha it is Odia, in Maharashtra it is Marathi. In Madhya Pradesh it is right.

Section 3(1) of the Madhya Pradesh Official Language Act, 1957, which received assent on 24 January 1958, provides that Hindi shall be the official language of the State for all purposes except such purposes as are specifically excluded by the Constitution. Hindi is spoken by well over two thirds of the state. So a Hindi daily and an English daily, both circulating in your own district, is the correct pair, and it is the pair the Department of Publication expects to see as two complete original pages.

Choose the edition, not just the masthead. A national daily printed in Delhi does not satisfy the point of the notice, which is that people who might object can actually see it. Book the Bhopal, Indore, Gwalior or Jabalpur edition that reaches your district, keep the complete printed page rather than a cutting, and make sure the date is clearly readable.

How does the name change procedure in Madhya Pradesh work, step by step?

Five stages. Nothing on this list needs you to travel, and nothing needs an agent to submit for you.

Step 1: what must the affidavit say?

Get an affidavit drafted on stamp paper stating your old name, your new name, your father's or husband's name, your full address in Madhya Pradesh, and a clear reason for the change, and sign it before a notary. Stamp paper and the notary fee together usually cost Rs 100 to Rs 500. Settle the exact spelling and spacing of the new name before this document is drafted, because everything after it has to match letter for letter. For a minor, the parent or legal guardian makes the declaration with their own details.

Step 2: which newspapers, and what must the notice carry?

Publish the same notice in two daily newspapers that genuinely circulate where you live: one Hindi daily and one English daily, in the edition that reaches your district. The notice must carry your old name, your new name, your full address, your age, your father's or husband's name, and the date of the affidavit, in exactly the form used on the affidavit. Keep the complete original printed page of each newspaper, not a cutting, with the date clearly visible. Two notices usually cost Rs 800 to Rs 4,000.

Step 3: what goes into the Delhi file?

Your file should contain the original newspaper pages carrying both notices, a signed undertaking showing your old and new name with parentage and residential address and declaring the facts are true, two copies of the prescribed proforma, computer typed and not handwritten, signed and attested, a soft copy of the notice text as the department directs, a certificate signed by you declaring that the hard copy and soft copy contents match, self-attested identity proof of the applicant and of one witness, two passport size photographs, and a request letter to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054.

Keep every supporting document recent. The Department expects papers that are not older than one year.

Step 4: how do you pay the fee?

The fee is paid only through bharatkosh.gov.in, the Government's Non-Tax Receipt Portal. It is commonly Rs 1,100 for an adult and Rs 1,700 for a minor, with an additional charge per alias name if you publish aliases. The Department of Publication revises these amounts each financial year, so confirm the current figure on deptpub.gov.in before you pay. The fee is not refundable. Do not arrange a demand draft in favour of any Controller of Publications in Bhopal or Madhya Pradesh: that officer is in Delhi, and the demand draft route closed years ago.

Step 5: where do you send it, and how do you get your copy?

Send the complete signed file by post or courier to the Department of Publication, Civil Lines, Delhi 110054. Speed post from Bhopal works exactly as well as walking it in from a Delhi address, and costs Rs 50 to Rs 200. Applications are accepted from the applicant directly, not from agents, which is why an honest consultant prepares and checks your file end to end while you sign and send it.

Your notification then appears in Part IV of the Gazette of India, in weekly Saturday dated issues, about 30 to 45 days after the file reaches Delhi. Physical printing and sale of hard copies ceased on 1 October 2015, so you open the relevant issue on egazette.gov.in, search the PDF for your old or new name, and download the whole gazette as your own copy. It needs no further certification.

Part IV belongs to the Central Gazette only. It is the section carrying notices from private people rather than from Government. A state gazette has its own numbering and its own parts, so any page telling you your notification will appear in Part IV of the Gazette of Madhya Pradesh has confused the two documents.

Do you need police verification, a Tahsildar certificate or a tatkal fee?

No, no and no. Three requirements circulate in the Madhya Pradesh results that will cost you weeks if you chase them.

Police verification

No gazette application asks for one. Pages describing neighbour enquiries and police clearance for a name change are describing an entirely different procedure, not your Central Gazette file.

A Tahsildar certificate

A citizenship or residence certificate from the Tahsildar is not required, and is not something you should be collecting before applying. There is no state level clearance in this process at all.

A tatkal gazette

There is no official expedited service. A company can only speed up its own paperwork, not the Department's publication schedule. A promise of publication in ten to fifteen days is selling you their queue.

What genuinely delays files is far more boring: a mismatch between the affidavit and the newspaper text, a cutting instead of a full page, a soft copy that does not match the hard copy, or documents older than a year. None of those cost money to avoid, which is the point.

The demand draft is the fourth thing to drop. The Department of Publication takes payment only through bharatkosh.gov.in. Any checklist asking you to make a draft payable to a Controller of Publications, Madhya Pradesh or Controller of Publications, Bhopal is describing an office that does not exist, using a payment route that closed years ago.

What documents are required for a name change in Madhya Pradesh?

What else applies to your case?

  • Divorce decree, if the change follows a divorce
  • Birth certificate of the child, where the applicant is a minor
  • Deed attested by the Indian Embassy or Consulate, if you apply from abroad
  • Departmental deed, if you are a Central Government employee
What you do not need: a demand draft, a deed poll, a police verification, a Tahsildar certificate, or a lawyer. Every one of those appears on Madhya Pradesh pages, and none of them is part of the file.
One written form, everywhere. Before anything is printed, fix the spelling, the middle name treatment and whether an initial is expanded. To a Government database, Chouhan and Chauhan are two different people. We put the final wording in writing with you before the affidavit is typed.

Who can apply, and what you need

Any Indian citizen at any stage of life: a husband or a wife independently, a parent or legal guardian for a minor, a Madhya Pradesh resident living in another state, or an applicant abroad through an attested deed. You need Indian citizenship, a valid Government photo identity, and a stated reason written into the affidavit. What you can change is equally broad: the full name, the spelling, the surname, or a middle name. The affidavit states exactly what the old full name was and exactly what the new full name will be.

Two pricing claims in the Madhya Pradesh results deserve a straight answer. The Rs 999 all-inclusive offers cannot be real, because the Government fee alone is commonly Rs 1,100. And a separate figure doing the rounds, that a name correction costs a flat Rs 1,400, does not match the published structure either: a correction of a name is handled as a name change under the same fee heads. Ask anyone quoting you a price two questions: which gazette, and does that include the fee paid on BharatKosh.

One more thing worth knowing before you commit. The Government fee is not refundable. If the file comes back because the surname was spelled one way on the affidavit and another way on a notice, you pay it again and wait again. That is the entire reason we settle the exact wording in writing before anything is drafted.

Plan for 3 to 7 weeks from the first phone call to a notification you can download. Distance adds a few postal days at most, so a file from Rewa is not meaningfully slower than one from Delhi.

There is no official expedited option. Treat any promise of faster publication for an extra charge as a warning about the seller, not a service. If a passport appointment, an exam form or a joining date is behind your file, begin 3 to 7 weeks ahead instead of paying for speed nobody can actually sell you.

The words that appear on every name change website, in language people actually use.

Gazette notification
A formal announcement printed in the official journal of a Government. For a name change it is the document Aadhaar, PAN, passport offices, banks, boards and employers accept as legal proof.
Central Gazette
The Gazette of India, published by the Department of Publication in Civil Lines, Delhi. Accepted everywhere in India and by foreign missions, and the route open to a Madhya Pradesh citizen.
Part IV
The section of the Gazette of India carrying notices from private people rather than from Government. Personal name change notifications appear here, and Part IV belongs to the Central Gazette only.
Controller of Publications
The officer who heads the Department of Publication, at Civil Lines, Delhi 110054. Your request letter is addressed to this officer, who is not in Bhopal and not in any state capital.
Affidavit
A written statement you swear is true, signed before a notary. It records your old name, your new name, your parentage, your address and the reason for the change.
Proforma
The prescribed application format the Department of Publication requires, computer typed in two signed and attested copies with the rest of the file.
Matching-contents certificate
A short signed statement that the hard copy and the soft copy of your notice text are identical. A mismatch here is a common cause of a file coming back.
BharatKosh
The Non-Tax Receipt Portal at bharatkosh.gov.in, the only accepted channel for the Central Gazette fee. Demand drafts and direct bank transfers were discontinued.
e-Gazette
The portal at egazette.gov.in where published Central notifications are searched and downloaded free. A download portal only, and the PDF is the official copy.
Registrar correction
A correction to a birth or death record under Section 15 of the Registration of Births and Deaths Act, 1969, through the body that issued the certificate. Not a gazette matter.
Nagar Nigam
A municipal corporation. In Madhya Pradesh it is the body that registers births and deaths in a city, and therefore the body that corrects them.
Deed poll
A British instrument for changing a name. India does not use it. The Indian equivalent is the affidavit, newspaper and gazette route on this page.

What do you update after the gazette, and in what order?

The gazette is the middle of the journey, not the end. Doing this in the wrong order turns a four week job into a four month one.

Is a birth certificate mistake a gazette matter in Madhya Pradesh?

No, and this is one of the most common things people here are sold wrongly. If the error sits in a birth certificate, a wrong spelling, a wrong parent's name or a missing child's name, the fix is a Registrar correction under Section 15 of the Registration of Births and Deaths Act, 1969, through the body that issued it: your Nagar Nigam, Nagar Palika Parishad or gram panchayat.

A gazette notification will not change the entry in the birth register. The two live in different systems, and the Civil Registration System portal is crsorgi.gov.in. Pages telling MP readers that a birth certificate name correction must be published in the State Gazette have the process wrong, and agents who push every birth certificate case into a gazette application are charging you for the wrong thing. Which special situations come up most in Madhya Pradesh?

Six cases we see constantly, starting with the two that are specific to this state.

Hindi to English spelling mismatches

The most common real case in the state. The same Hindi name reaches English differently across decades of records: Chouhan and Chauhan, Shukla and Shukl, Kumari present on a school certificate and missing from the Aadhaar, a middle name written in full on one paper and dropped on another. To Government systems these are different people, and one mismatch blocks a passport, a bank KYC or an exam form. A gazette notification declaring one final spelling fixes it at the root instead of one document at a time.

Documents from before the Chhattisgarh split

Anyone schooled or employed before 1 November 2000 in Raipur, Bilaspur, Durg or the wider Chhattisgarh region holds certificates issued by Madhya Pradesh. Those documents stay valid: the Madhya Pradesh Reorganisation Act, 2000 carried the existing laws forward into the new state, and nothing in it invalidates a certificate already issued. A certificate naming MP does not become wrong because the district now sits in another state. Where the name itself differs across older and newer records, a gazette notification gives every office one version, and Part IV is valid in both states.

MP Board and university records

The Board of Secondary Education, Madhya Pradesh at Bhopal, set up under the Madhya Pradesh Secondary Education Act, 1965, runs its own correction process for names on marksheets and certificates, and universities have theirs. The gazette notification supports that application, especially for a bigger change, but the board's own procedure decides the outcome and charges its own fee. Nobody can promise that a gazette alone rewrites a board certificate.

Marriage, divorce and a minor child

A woman adding a husband's surname follows the same three stages with the marriage certificate attached, and reverting to a maiden name after a divorce follows the same route with the decree attached. If you simply keep using your existing name and change nothing on paper, no gazette is needed at all. For a minor, a parent or legal guardian applies and swears the affidavit, attaching the child's birth certificate, and the minor fee applies, commonly Rs 1,700 with the usual revision caveat. Both parents signing avoids objections later.

Applying from abroad. Execute the deed, have it attested by the Indian Embassy or Consulate of that country, and apply to the Central Gazette, budgeting around Rs 7,500 to Rs 9,000. Family in Madhya Pradesh can post the file for you, but the papers must carry your own signature, and the final notification downloads from egazette.gov.in anywhere in the world.

What changes for Government employees in Madhya Pradesh?

Central Government employees follow the Ministry of Home Affairs Office Memorandum No. 190016/1/87-Estt.(A) dated 12 March 1987: a deed of change of name is executed and routed through the department, so that the service book is updated alongside the gazette notification.

State Government employees should likewise inform their department rather than doing the gazette quietly. A service record that does not match the published name creates a second problem, usually at promotion or pension, years later.
